FDA approves first size-adjustable heart valve for children

How the size-adjustable heart valve works

The Autus valve can initially be implanted at approximately 13 millimeters in diameter, a size the FDA describes as suitable for a toddler or preschool-aged child. It can later be widened using a balloon catheter, a small inflatable tube, to reach up to 22 millimeters—comparable to an adult-sized pulmonary valve.

The device does not grow automatically. Its design allows expansion after implantation. This distinction matters because the potential benefit is accommodating a child’s growth without replacing the valve through another open-heart operation.

The valve’s leaflets—the inner flaps that open and close to control blood flow—are made from polymeric material. The FDA says animal-derived tissue used in traditional valves tends to harden and break down faster in children. One-year follow-up data suggest the Autus valve’s synthetic leaflets hold up well, although longer-term research continues.

Who may be affected by the approval?

The approval concerns children with congenital pulmonary valve disease who need pulmonary valve replacement. Pulmonary valve stenosis involves a narrowed valve, while pulmonary valve atresia involves a valve that does not form normally. Both can restrict blood flow from the heart to the lungs.

The FDA reports that congenital heart defects affect about one in every 100 U.S. births. Citing the Centers for Disease Control and Prevention, the announcement says pulmonary valve atresia and stenosis together affect approximately 4,200 babies in the United States each year.

Some children may have significant pulmonary regurgitation, or backward leakage through the valve, after treatment for these defects. Over time, that leakage can weaken or enlarge the right ventricle and create a need for valve replacement. The announcement does not describe this device as an option for every congenital heart defect.

What the clinical study found

The FDA based its approval on a study enrolling 62 pediatric patients at 12 U.S. sites. At six months, the first 60 patients had acceptable blood-flow performance through the right side of the heart and no more than mild leakage through the valve.

All study patients had a successful procedure. The FDA reported no deaths, strokes, blood clots requiring additional treatment, or other clot-related complications. Three patients experienced valve-frame fractures, and two had reduced movement of one leaflet. None of those events caused symptoms.

Valve performance began deteriorating in two patients who started to outgrow the device. Both valves were successfully expanded without an invasive surgical procedure. However, the FDA emphasized that experience with expansion is limited. Longer-term studies are examining risks and whether expansion can maintain valve function and reduce surgical procedures.

The device received approval through the FDA’s premarket approval pathway, its most stringent medical-device marketing application process. Edwards Lifesciences received approval for the valve, which was developed by Autus Valve Technologies Inc.
